If you are a step-parent, grandparent or other relative of the child you wish to adopt, the Greenlee County Attorney's Office will provide you with the following forms:
- Adoption Information
This form provides our office with all the information we need to complete the Petition for Adoption and Request for New Birth Certificate.
- Consent to Adoption
This form, signed by the natural mother or natural father relinquishes her/his parental rights and gives consent for the child to be adopted. (This form must be signed in front of a Notary Public.)
- Petition for Adoption and Acceptance and Waiver of Process
This form, signed by the natural mother or natural father relinquishes her/his parental rights and gives consent for the child to be adopted. (This form must be signed in front of a Notary Public.)
- Notice of Hearing Petition for Adoption
The Judge will set a date for the Final Adoption Hearing, usually within 60 days of the date the Petition is filed. If a date is set and you, the adoptive parents, cannot be at the courthouse on that date or time, please send a letter to the Clerk of the Superior Court, indicating that you would like the date to be moved (you may indicate choices of a better time and date)
- Final Adoption
This hearing is held in the Superior Courtroom on the second floor of the Courthouse. The County Attorney and Judge may ask you a few questions regarding your desire to adopt. This proceeding will take about 30 minutes.
Note: We will need a copy of the Child's original birth certificate prior to the Final Adoption hearing.
- Request for New Birth Certificate
We will prepare a Request for a New Birth Certificate for you, the adoptive parents, and the Clerk of the Court to sign. Unless the child was born in a state other than Arizona, we will mail the request to the Arizona Dept of Vital Statistics and a new birth certificate will be mailed to you. If the child was born in another state, you will need to contact that state's Vital Statistics department for information on obtaining a new certificate.
